195 Small bowel obstruction through peritoneal defect herniation after a totally extraperitoneal hernia repair- A unusual complication case series

Abstract The rates of major complications associated with totally extraperitoneal (TEP) inguinal hernia repair are reportedly low, and intra-abdominal rare. However, here we present a case series small bowel obstruction through herniation the into peritoneal defect after TEP, which is rare complication. first case, 71year old male two weeks post-TEP left repair, presented abdomen pain, vomiting, distension, bowels not opened for three days elevated inflammatory markers. Computed Tomography pelvis (CTAP) demonstrated secondary to loop inflamed adherent anterior abdominal wall in region. During emergency diagnostic laparoscopy found have passing just lateral medial umbilical ligament, converted laparotomy, resection was performed. In second 61-year-old similar complaints markers post bilateral TEP. CTAP showed dilated transition point iliac fossa. Emergency on groin herniation. bowel-reduced back closed sutures. Small due TEP but potentially fatal Therefore, clinician should low threshold surgery, as it unlikely resolve conservative treatment like adhesional obstruction.
